Overview

The Community Health Director will be responsible for the planning and implementation of community health outreach strategies through delivery of care, education and support that address specific disparities and barriers to care across all the communities served by Atrium Health Floyd.

Responsibilities:

  • Collaborate with other departments and teams to identify, plan, develop and execute strategies address disparities and barriers to care.
  • Develop and nurture relationships with collaborative partners in the community.
  • Coordinate local programs and initiatives.
  • Help implement enterprise Social Impact strategies.
  • Coordinate required Community Health Needs Assessment & Community Benefit Reports
  • Maintain a calendar of community health outreach efforts.
  • Prepare an annual budget for community health outreach activities.
  • Schedule regular outreach initiatives and educate teammates on community responsibility.
  • Prepare accurate records and report on the goals of initiatives.
  • Report at least annually to Community Benefit Committee of the FHMI Board.
  • Help implement enterprise Acute Social Needs strategies, including oversite of the Community Resource Hub

 

Requirements:

Master of Business Administration required as well as a Bachelor’s Degree in a related area of study. A minimum of 5 years of experience specializing in community outreach including the development of strategy through implementation. A thorough understanding of community health and DEI principles, practices, and procedures. Experience in health and education is a plus. Ability to work weekends and evenings as needed.
