FLSA Status: Exempt Original Date: November 2011 Last Revision: September 2018 JOB SUMMARY: Coordinates the Outpatient Diabetes Education Program for the center. Collaborates with a multidisciplinary team in determining age-appropriate educational needs of patients with diabetes. Oversees the development of appropriate educational materials, and plans outpatient programs such as comprehensive diabetes education programs, support groups, staff development, diabetes in-services and orientation for new staff, as well as other hospital staff. EDUCATION/EXPERIENCE: Associate's degree (or Two-Year Professional Program) from an accredited school of Nursing required. BSN or Bachelor's degree in Human Nutrition, Foods and Nutrition, Biological Sciences or Dietetics with three years' experience in diabetes care, education program management and chronic disease preferred. Broad range of knowledge on diabetes for age appropriate population and its interrelation to co-existing health problems required. Master's degree preferred. Leadership and supervisory experience. LICENSURE, CERTIFICATION, and/or REGISTRATION: Current license to practice in North Carolina as a Registered Nurse or Registered Dietitian. Certified Diabetes Educator (CDE) or Board Certified- Advanced Diabetes Management (BC-ADM) required or actively achieving eligibility within two years. BCLS required. ESSENTIAL FUNCTIONS: 1. Educates and counsels diabetes patients. 2. Serves as a liaison between the advisory group and the institution?s administrator. 3. Coordinates and participates in the orientation of diabetes patient education personnel. 4. Assesses staff training needs, staff development, coordinates, and provides in-service education as needed. 5. Coordinates the program curriculum and communicates with the Program Managers in Boston regarding the center's needs. 6. Prepares the required audits and assists in development of a CQI plan. 7. Evaluates the program's content and effectiveness. 8. Keeps current in the latest diabetes care and educational techniques and products available in order to maintain a state of the art approach. 9. Develops and coordinates education program strategic plan; documents accomplishments, evaluates and reassess priorities. 10. Assists in budget development and staff performance evaluations as directed for program. 11. Monitors compliance to all safety, infection control, HIPAA, Joint Commission, Point of Care regulations. 12. Assists in development of community/professional continuing education as needed. 13. Coordinates application for ADA Recognition or AADE Accreditation and yearly updates. Chairs the Diabetes Education Advisory Committee and is responsible for coordinating the annual review process. Promotes the Joslin Education Program by serving as a spokesperson for the program. Functions as an educator (as described in the nursing and dietitian job descriptions).
