Overview

The Capacity Case Manager assesses, plans, coordinates, and evaluates the daily clinical and operational functions and needs of the organization by working closely with Patient Care Services Leadership to ensure quality cost-effective healthcare is utilized by the Interdisciplinary Teams. Promotes a safe environment and performs all related job responsibilities in a safe manner. Maintains clinical and professional competencies as appropriate for the age, developmental stages, and special needs of patient’s served. Serves as a clinical resource for the Throughput Navigation Coordinators and assist with coordinating, prioritizing, and optimizing patient flow using knowledge of clinical and operational activities throughout the hospital network. 

Shift details: This position is 9A - 5:30P with weekend rotation.

What you will need:

  • Must be a graduate of an approved school of professional nursing currently licensed to practice as a Registered Nurse with the Georgia Board of Nursing. 
  • BSN required; ASN with six (6) years nursing experience may substitute for BSN.
  • Minimum of three (3) years’ experience in Acute Care Hospital setting preferably Emergency Room or ICU experience.
  • Knowledge of pre-certification, case management, level of care screening preferred.
  • Experience with IT solutions such as electronic medical record, learning management or disease/care management systems a plus.
  • Critical thinking skills and ability to analyze complex data sets required.
  • Strong communication, decision-making, and problem-solving skills.
  • Excellent time management and organizational skills.
  • Ability to handle changing work environment and to prioritize activities.
