Overview

Job Summary

Develops plans and implements community-based health programs aimed at the prevention of disease and promotion of health. Provides leadership of multi-disciplinary teams within a broader Community Health infrastructure. Responsibilities include overseeing, developing, and supporting Community Health programs which achieve outcomes consistent with the organization’s mission and strategic priorities. Responsible for identifying and providing Community Health support to internal and external partners across the Atrium Health enterprise.



Essential Functions

  • Manages specific Community Health programs from planning and design through implementation and evaluation.
  • Manages the advancement of the work and initiatives within the department by collaborating with other clinical and administrative leaders, achieve goals and objectives.
  • Identifies and facilitates connections between system strategic priority work, service line work, regional and other partnership work, and operational level work to advance the system strategy and vision.
  • Assist in advancing the growth and value of the Community Health capability in a scalable and value-added way to serve as a national model.
  • Develops a network of support within and outside of the system and organizes competent group facilitation to achieve organizational objectives.
  • Assess and manage community needs across the enterprise.
  • Collaborates with internal partners/leaders and may facilitate primary enterprise or system-wide teams focused on Community Health interventions.
  • Works in partnership with AVP to maintain fiduciary responsibility for Community Health budget and provide insight on strategic direction for the department.
  • Implements mechanisms to enhance ability to track, measure, document and assess impact and outcomes for Community Health related interventions. Implements mechanisms to enhance ability to track, measure, document and assess impact and outcomes for Community Health related interventions.
  • Developing and maintaining relationships with internal and external Community Health partners.

Physical Requirements

Work requires walking, standing and sitting. Works in an office environment. Intact sense of sight and hearing. Must be able to lift 10 pounds of weight. Work requires critical thinking and ability to concentrate. Must be able to respond quickly in stressful situations. Must be able to write and speak English in a clear, grammatically correct manner.



Education, Experience and Certifications

Bachelor’s degree required. Master’s degree in preferred, human service or health related field a plus. Minimum of 5 years of leadership experience preferred. Knowledgeable of the surrounding community and associated demographics helpful. Excellent written and verbal communication skills.
