Overview

TRANSPORT COORDINATOR (MATERNAL/NEONATAL)

 

Coordinates all data and reporting as outlined in the Maternal and Infant Care Contract. Coordinates all maternal data for National Perinatal Information Center (NPIC). Works collaboratively with DPH and GaPQC on state quality improvement projects. As Grant Coordinator, responsible for coordinating allocation of maternal benefits funds to eligible parties in accordance to the guidelines set forth by Maternal and Infant Care Contract. This process includes, but is not limited to, verifying patient data, maintaining files, and tracking and follow up in Sorian financial system. Interacts regularly with Navicent’s financial business office, information services, and health information departments related to the IIC contracts, compliance and accuracy of grant information. Must be able to work autonomously, with good organizational skills and a high degree of confidentiality in the management of high risk patient data.

Essential Functions and Responsibilities

· Grant Coordinator Specifics:

1. Tracking of all obstetrical patients on a daily, monthly basis

2. Establish patient’s high risk criteria, verify medical eligibility.

3. Provide the grant applications to potential applicants and explain the purpose of the grant funds as identified by Grant criteria

4. Follow up and track the completion of the applications.

5. Verify financial eligibility in accordance to the Federal Poverty Level Guidelines.

6. Maintain grant files.

7. Responsible for approving allocation of grant funds to patient bills.

8. Responsible for coordinating and submitting all financial accounting procedures and financial audit requirements to organization’s financial department.

9. Responsible for updating grant letters annually to reflect changes to the Federal Poverty Level Guidelines and required documentation.

10. Responsible for updating Verification of Medical and Financial Eligibility Policy as needed.

o Collects, prepares, and submits required quarterly programmatic data to the Department of Public Health in accordance to the Maternal and Infant Care Contract.

o Responsible for coordinating collection and submission of required data for annual report to National Perinatal Information Center (NPIC).

o Review quarterly NPIC reports with Maternal and Neonatal Outreach Coordinators and update appropriate parties, i.e., Women’s services leadership, OB/GYN physicians, QMS.

o Attend weekly HROB Clinics and maintain monthly log of patients seen in HROB Clinic.

o Develop and maintain a data base for annual information required under M & I Care contract with Georgia Department of Public Health.

o Participates in the annual Georgia DPH annual audit

o Serves as primary liaison and project lead on Quality Improvement initiatives as set forth by DPH and Georgia Perinatal Quality Collaborative (GaPQC).

o Coordinate implementation of QI projects.

o Responsible for coordinating GaPQC QI data collection and submission quarterly as requested by GaPQC.

o Collaborate with Women’s Services leadership on implementation of QI initiatives.

o Review and disseminate quarterly report information to appropriate parties.

o Ongoing review of projects with Maternal Outreach Coordinator.

o Assists with OB M&M committee case abstraction, review, and data collection.

o Serves on assigned MESO committees related to Perinatal QI projects.

 

Education, Training, and Experience

o Current license as Registered Nurse by the Georgia Board of Nursing and certification in Inpatient Obstetric Nursing or related specialty.
