The Community Intervention Specialist will provide acute intervention, outreach/advocacy, counseling and case management for victims of interpersonal violence. The position requires the ability to provide clinical services in a non-traditional manner/setting, utilizing a wide range of strategies to address symptoms of trauma and disruptions in daily living (safety, emotional management, loss, and future). The candidate must also be able to provide case management services, and to support patients in the program in increasing connections to employment, family, social supports, and social services. Job requires a significant amount of community/home-based work, and a willingness to engage within a diverse range of neighborhoods, partners and community members in Mecklenburg County, North Carolina and between stakeholders in Atrium Health’s Division of Community and Social Impact, Division of Acute Care Surgery, Trauma Services, and the City of Charlotte and Mecklenburg County Public Health.
Position requires a detail-oriented, organized, self-starter for the provision of acute intervention and follow-up services provided by the Atrium Health Violence Intervention Program (AH-VIP). AH-VIP is a trauma informed, hospital-based, community focused intervention strategy dedicated to reducing violent injuries, deaths, and the long-term consequences of trauma and to increasing physical and emotional healing among residents of Mecklenburg County, North Carolina. Position will have dual reporting to Community Health and Trauma Services.
· Provides trauma informed emotional support, mentoring, and advocacy to young people who are evaluated and treated for interpersonal violence-related injuries at Atrium Health-Main Medical Center.
· Provides referrals and intensive case management to assist in successful connection to community-based resources.
· Maintains intensive follow-up contact with clients, family, friends and service providers through in-person visits, virtual visits (video or telephone), and in-writing (e-mail, text, etc.)
· Utilizes trauma informed modalities and interventions with clients, including Motivational Interviewing, Child and Family Traumatic Stress Intervention, Trauma Art Narrative Therapy, CBT, CPT or the SELF model
· Keeps consistent and accurate records of all contacts with clients in web-based database, and provides documentation needed for billing for services
Generally, works in a hospital and office-based setting at Atrium Health facilities. Some of the time is spent in a community setting, meeting with external partners. On occasion, requires standing, walking and traveling in a personal car to other facilities. Also, occasionally requires carrying up to 25 pounds of materials (files, projectors, laptops). Position also requires strong communication skills as needed to build relationships with patients and work across multiple departments and external partners.
Education, Experience and Certifications
A high school degree or GED equivalent is required, a Bachelor’s degree in social work, public health, health education, community development, human services, education, or public administration is preferred. Must have a minimum of 2-3 years of experience in the healthcare sector, non-profit sector, education or those with comparable work experience. Prior experience with Microsoft Office suite of tools is required. Applicants with work experience in trauma services/trauma informed care are encouraged to apply. Experience working with disadvantaged people and communities, the ability to understand and navigate communities of color, and the ability to quickly establish credibility with individuals from a broad social-economic patient population, including patients with criminal backgrounds, gang involvement, substance use is encouraged.
