Overview
Case Manager ensures patient’s progression through the continuum of care in a manner that achieves the desired clinical and financial outcomes. Provides leadership, monitoring, clinical expertise and functional direction to other care team partners/providers. Provides care consistent with ages of the population served. This position is day shift, Monday - Friday with On call weekends.
What you will need:
- Graduate of an accredited school of nursing, BSN or MSN preferred
- Three (3) to five (5) years experience in hospital setting; prefer one (1) year medical rehabilitation experience. Prefer charge nurse and case management experience.
- Current licensure as a professional registered nurse by the State of Georgia. Certification in rehabilitation nursing (CRRN) preferred
- Current certification in CPR.
