Job Summary:
Seeking clinical pharmacist to join virtual population health pharmacy team to provide quality-oriented medication management services to ensure that patients within value-based contracts receive optimal patient care while achieving the highest performance on payors annual agreements. This role includes working with patients and physicians to help manage chronic disease as well as improve medication adherence, medication access, and other quality metrics.
Job Requirements:
Pharm.D., M.S., or B.S. Degree.
Valid Georgia Pharmacist License.
Residency and/or board certification strongly preferred.
Strong communication skills, prior ambulatory experience, and knowledge of third-party contracts preferred.
Must reside in the state of Georgia or be willing to relocate.
