Overview

Job Title: Pediatric Nephrology Social Worker

Location: Brenner Children’s Hospital

Job Summary: We are seeking a dedicated and compassionate Pediatric Nephrology Social Worker to join our team and help us develop and build a psycho-social support program. This full-time position focuses on providing coordinated services to infants, children, and youth living with kidney disease, including macro social work. The social worker will play a vital role in our multi-disciplinary care team, ensuring seamless communication and care coordination among healthcare providers, patients, and their families. We are very open to tailoring the position to an individual’s strengths and interests within macro social work as they work with us to build a vision for their role.

Key Responsibilities:

  • Care Coordination: Schedule clinic appointments, manage changes in foster care or group homes, address barriers to care, coordinate care with other specialties.
  • Mental Health Support: Assist patients in receiving mental health services and organize support groups for children with kidney disease and their parents. Assess and diagnose mental health conditions and develop individualized treatment plans. Provide individual, group, and/or family therapy. 
  • Community Outreach: Encourage involvement in kidney disease community outreach initiatives.
  • Emotional Support: Offer a safe space for patients and families to express their feelings and provide coping strategies.
  • Education and Empowerment: Coordinate educational sessions on kidney care self-management, healthy lifestyle choices, and transition of care topics.  
  • Psychosocial Assessment: Identify psychological or social concerns impacting kidney disease management and devise personalized care plans, including supporting families with education planning.
  • Community Resources: Connect families with community resources, support groups, and financial assistance programs.
  • Transition to Adulthood: Work with our team to build a transition of care program and assist adolescents in transitioning to adult healthcare services.

Qualifications:

  • Master’s degree in Social Work (MSW) from an accredited institution
  • Licensed Clinical Social Worker (LCSW) or equivalent certification.
  • Experience in pediatric healthcare, preferably in nephrology.
  • Strong organizational and communication skills.
  • Ability to work collaboratively with a multidisciplinary team.
