Overview
Primary Purpose
Coordinates and delivers Diabetes Education for pediatric patients and family members. Collaborates with a multidisciplinary team in determining age-appropriate educational needs of patients with diabetes. Participates in the development of appropriate educational materials and plans programs such as comprehensive diabetes education programs, support groups, staff development, diabetes in-services and orientation for new teammates, as well as other hospital staff. In collaboration with the medical staff and nursing staff, monitors the clinical care and education of patients and provides diabetes educational support to improve patient care and patient outcomes. Acts as a role model, patient advocate, change agent, leader and cost-effective practitioner. Maintains Certified Diabetes Educator or Board Certified - Advanced Diabetes Management certification and other required licensure.
Major Responsibilities
Evaluates, educates, and counsels pediatric diabetes patients and family members
Uses established principles of teaching-learning theory and life-style counseling.
Individualizes instructions for persons of all ages, incorporating their cultural preferences, health beliefs and learning styles.
Evaluates patient's/family's response to education, treatment and health services provided.
Documents patient assessment, education, learning barriers and interventions to address barriers, and patient response to education.
Serves as a resource to nursing, clinical, and other healthcare personnel and the community.
Coordinates program curriculum and communicates with Diabetes Education Program Manager & Service Line leadership on program needs.
Evaluates the program’s content and effectiveness and proactively introduces improvements as needed.
Maintains current knowledge of the latest diabetes care educational techniques and products available in order to maintain a state-of-the-art approach.
Follows guidelines of the American Diabetes Association (ADA) and incorporates best practices in treatment plans
Maintains compliance to all safety, infection control, HIPAA, Joint Commission, ADA, & Point of Care regulations.
Assists in development of community/professional continuing education as needed.
Minimum Job Requirements
Education: Degree from an Accredited School of Nursing OR Bachelor’s Degree in Human Nutrition, Food and Nutrition, Biological Sciences, or Dietetics required.
Certification / Registration / License: Current license to practice as a Registered Nurse (RN) OR Registered Dietitian in the State of North Carolina. Certified Diabetes Educator (CDE) or Board Certified - Advanced Diabetes Management (BC-ADM) required or actively achieving eligibility within two years of hire. BLS Required per policy guidelines.
Work Experience: Two years of experience in diabetes care, education management, chronic disease or related patient care required.

Physical Requirements and Working Conditions
This position requires walking, sitting, standing, reaching, bending, pushing and pulling. Must be able to list and support the weight of 35 pounds. Must have intact sense of sight, hearing, smell, touch, and finger dexterity. Possible exposure to infections and contagious diseases. Possible/occasional exposure to patient illnesses.
