Overview

JOB SUMMARY

Provide assessment and counseling services to prospective and current enrollees of the GUIDE program, a CMS demonstration project for patients with dementia and their families.   This position will perform assessments essential to determining eligibility for GUIDE. In addition, clinical interventions include conducting family meetings and individual, family and group counseling with patients and families who present with a variety of behavioral health needs in the context of a dementia diagnosis in the family.  In conjunction with the multi-disciplinary team, assists with the development and implementation of a comprehensive treatment plan to meet ongoing health and mental health needs. May also coordinate with other community service providers. 

EDUCATION/EXPERIENCE

Master's degree in Social Work, Counseling or Marriage and Family Therapy required. 

LICENSURE, CERTIFICATION, and/or REGISTRATION

North Carolina Licensed Clinical Social Worker, or North Carolina Licensed Psychologist or NC Licensed Professional Counselor or Licensed Marriage and Family Therapist preferred. 

ESSENTIAL FUNCTIONS 

  1. Collaborates with Gerontology team members to provide holistic care for patients. 
  2. Develops, manages, and maintains information for GUIDE Program tracking.
  3. Assists with cognitive screenings of prospective and enrolled GUIDE patients. 
  4. Identifies and accesses local dementia care resources for patients and caregivers. 
  5. Identifies and links patients with community programs to meet housing/nutrition/transportation needs. 
  6. Provides brief counseling for appropriate patients. 
  7. Provides group visits/ support groups for clients. 
  8. Acts as dementia care team resource to local agencies. 
  9. Maintains database of identified patient needs and resources. 

SKILLS/QUALIFICATIONS

  • Knowledge of the principles and techniques of administrative management
  • Strong organization, planning, staffing, training, budgeting, and reporting skills
  • Moderate proficiency in Excel/Microsoft Word
  • Familiarity with Electronic Medical Records, and integrated behavioral billing in primary care
