Overview
Job Summary:
Coordinates the clinical teaching/learning aspect of the diabetes program for Diabetes Healthways (DH). Activities include patient and family assessment and counseling regarding diabetes and its management, as well as teaching on an individual or group basis. May teach all components of the DH program as needed, including nutrition and medication management once cross trained. Acts as a case manager for assigned patients with diabetes, assessing patient and family needs regarding diabetes management and education, and coordinates their treatment plan with physicians and other treatment team members
Essential Functions:
• Effectively facilitates/coordinates the provision of DH services for assigned patients.
• Facilitates learning by patients, families and significant others regarding diabetes management.
• Serves as a resource for the DH team, hospital and community regarding DM management.
• Demonstrates self direction in fostering personal expertise and future professional development.
Education, Training, and Experience:
BS degree in Nursing or B.S. Degree in Dietetics or Food & Nutrition; Master’s degree preferred. Two years of clinical experience, preferably in diabetes patient education and supervision.
