JOB SUMMARY: Coordinates the Outpatient Diabetes Education Program for the center. Collaborates with a multidisciplinary team in determining age-appropriate educational needs of patients with diabetes. Oversees the development of appropriate educational materials, and plans outpatient programs (such as comprehensive diabetes education programs, support groups, staff development, diabetes in-services and orientation for new staff, as well as other hospital staff). Is responsible for coordinating application for ADA Recognition or AADE Accreditation and yearly updates. Chairs the Diabetes Education Advisory Committee and is responsible for coordinating the annual review process. Promotes the Diabetes Education Program by serving as a spokesperson for the program. Functions as an educator (as described in the nursing and dietitian job descriptions).
EDUCATION/EXPERIENCE: Graduation with a BSN from an accredited School of Nursing and three years’ experience in diabetes care, education management and chronic disease required; or, Bachelor’s degree in Human Nutrition, Foods and Nutrition, Biological Sciences, or Dietetics. Master’s degree preferred and a broad range of knowledge on diabetes for age appropriate population and its interrelation to co-existing health problems. Has academic preparation and/or experiential preparation in program management, and care of persons with a chronic disease.
LICENSURE, CERTIFICATION, and/or REGISTRATION: Current license to practice as a Registered Nurse (RN) in the State of North Carolina or Registered Dietitian. Certified Diabetes Educator (CDE) or Board Certified- Advanced Diabetes Management (BC-ADM) required or actively achieving eligibility within two years of hire.
