Overview
Primary Purpose
The Patient Care Coordinator supports the medical groups efforts in population health management, as well as value-based contract performance, and community health outreach with available platform. This individual provides care organization, coordination, and management of the patient's individual care while also focusing on quality measures in value-based contracts. The Patient Care Coordinator II monitors the compliance of the value-based measures and reports that compliance to the PCP network while assisting the Primary Care offices and Population Health department to facilitate resolution of issues to achieve compliance.
Major Responsibilities
- Maintains competence in applicable licensure, certification, accreditation, and other regulatory body requirements.
- Proficiently facilitates performance improvement efforts and coaches’ others in the development of performance improvement capabilities.
- Analyzes trends of performance metrics/business unit level dashboards.
- Meet with providers and practice leaders on a regular basis to inform and educate on metrics and new processes.
- Coordinates care for new patients referred to primary-care providers by other network medical group providers.
- Outreach to patients for care gap closure via phone, letter, or electronic methods.
- Ensure chart documentation reflects care gap closure by performing chart audits on assigned practices. Educating practices and providers on opportunities found.
Minimum Job Requirements
Education
- High school diploma or GED from an accredited institute required.
- Medical Assistant, Licensed Practice Nursing or associate degree in social work required.
Certification / License
- Medical Assistant, Licensed Practice Nursing or associate degree in social work, or Registered Nurse required.
Work Experience
- One to three years of experience in a healthcare setting, such as a physician’s office, hospital department, or provider relations and a familiarity with standard quality measures.
- Experience with computer systems, including web-based applications such as Microsoft Office applications.
- Certification in health care quality, patient safety, or related accreditation entity preferred, required within 2 years of employment.
Knowledge / Skills / Abilities
- Experience with computer systems, including web-based applications such as Microsoft 365 applications.
- Verbal and written communication skills
Physical Requirements and Working Conditions
- Must have the ability to clearly communicate verbally, in person, and in writing with patients, families, agencies, and health care team. Works in an office environment. Requires long periods of sitting and computer use. Requires travel to assigned medical practice locations.
Preferred Job Requirements
Experience
- Three to Five years of related work
Knowledge / Skills / Abilities
- Extensive knowledge in utilizing Epic Electronic Heath Records, preferred.
- Knowledge of Lean concepts preferred.
- Intermediate to advanced computer skills with demonstration of skill and competence in use of all Microsoft Office applications preferred.
