Overview

JOB SUMMARY: 

Demonstrates mastery in the body of diabetes knowledge and treatment therapies while utilizing behavioral and case management interventions in the care of patients with diabetes. Provides quality diabetes self-management education (DSME) to all referred patients with diabetes and their families through the implementation of the American Diabetes Association (ADA) Recognition standards of care. Works with a multifaceted education instructional team using their individual disciplinary expertise to provide comprehensive age appropriate diabetes education. 

EDUCATION/EXPERIENCE: 

  • Graduation with a degree from an accredited School of Nursing with two years diabetes related work experience required. 
  • BSN or Bachelor's degree in Human Nutrition, Foods and Nutrition, Biological Sciences, Dietetics, Sociology, Public Health, Psychology, or Pharmacy preferred. 
  • Patient oriented educational experience preferred with a broad range of knowledge on diabetes for age appropriate population and its interrelation to co-existing health problems. 

LICENSURE, CERTIFICATION, and/or REGISTRATION: 

  • Current license or certification to practice as a Registered Nurse (RN) in the State of North Carolina, Registered Dietitian, Registered Pharmacist, Social Worker, Family Counselor, Health Educator, or Exercise Physiologist. 
  • Certified Diabetes Educator (C.D.E.) preferred. If not certified, must obtain certification within two years. 

ESSENTIAL FUNCTIONS: 

  • Maintains competency skills by participating in continued education and professional development. 
  • Provides patient management by assessing, planning and documenting. 
  • Manages intensive therapies that require critical thinking and independent judgment. Collaborates with the patient, referring physician, medical director, and other designated members of the health care team.
  • Implements interventions as per department guidelines, management protocols, and specific patient needs. 
  • Evaluates patient progress and revises plan of care as needed. 
  • Maintains extensive follow-up for complex patient population while exploring pertinent resources for improved outcomes.
  • Demonstrates proficiency in all aspects of diabetes education.
