Job Summary:
The Clinical Pharmacist II is responsible for providing pharmaceutical care to CGHS patients and functioning as the service representative for Pharmacy. Scope of duties includes, for responsible areas, overseeing pharmaceutical care delivery, ensuring pharmacy systems meet customer needs, and managing quality & financial performance.
Essential Functions:
Responsible for providing pharmaceutical care delivery to patients in assigned areas.
Develops pharmaceutical care plans, protocols, and care delivery meeting therapeutic goals and improving quality of life for each patient.
Performs consultations in pharmacokinetic dosing, pharmacotherapy, and parenteral nutrition, and other collaborative drug therapy practice as defined by department / organization policy.
Documents consult and follow up in medical record according to department / professional standards.
Monitors patient progress in terms of pharmaceutical care plan.
Intervenes as necessary to ensure safe and effective drug therapy outcomes.
Collaborates with prescribers and /or other care givers to address patient specific needs relating to drug treatment.
Documents pharmacist intervention results.
Supervises Clinical Pharmacist I in patient monitoring activities.
Participates in medical care rounds, transitional care (discharge planning) rounds, multi-disciplinary rounds and critical path development.
Functions as principal liaison to patient, family and other health care professionals regarding pharmaceutical care.
Responsible for pharmacy systems and financial performance for assigned areas.
Functions as pharmacy service representative to medical, nursing and other disciplines regarding pharmacy services.
