Care Coordinator Healthy Community - Palliative Care - Full time

Overview

Job Summary: 

Develops and facilitates a comprehensive care plan which includes medical services and social services post discharge with the primary goal of assisting patients to remain in their homes and in their communities. This would be short-term and long-term care plans. Works with the providers and caregivers to coordinate medical and social services for the patient and sometimes for the care givers and uses standardized tools to access their needs. Community outreach will be required. Will oversee the Community Health Workers in their area and provide support as needed. Will develop and provide educational programs may include, but not limited to, health screenings in the community specific to community healthcare needs.

Job requirements:

Bachelors’ degree in Social Work or Counseling and must be currently enrolled in Masters’ degree program in Social Work or Counseling and have completed first year of Masters’ program. Masters’ degree in Social Work or Counseling required within two years of hire (MSW, etc.).
