Overview
JOB SUMMARY: Assumes responsibility and accountability for the application of the nursing process and the delivery of patient care, under the supervision of a Registered Nurse (RN). Pro-vides age and developmentally appropriate direct patient care while consistently working and collaborating with assigned team members, directly reporting to the Registered nurse. Maintains accountability for patient care and assisting others in the management of patient care.
EDUCATION/EXPERIENCE: Graduation from Davidson County Community College Practical Nursing Program within the last 12 months. o Applicants are expected to complete the Associate Degree Nursing program pathway through DCCC o Pass the NCLEX-RN and obtain licensure as a Registered Nurse (RN) in the state of North Carolina
LICENSURE, CERTIFICATION, and/or REGISTRATION: Current Licensed Practical Nurse (LPN) in the State of North Carolina and Basic Cardiac Life Support (BCLS) certification required.
ESSENTIAL FUNCTIONS 1. Collects and interprets data directly related to patient care needs and in relation to the medical diagnosis. 2. Ability to determine immediate patient needs for intervention. 3. Utilizes the nursing process, in collaboration with the RN, to provide patient centered care focusing on the physical, emotional, spiritual, cultural, religious, and environmental needs. Includes providing collected data and interpretive input for the assessment, de-velopment of nursing diagnosis, outcome identification, planning, implementation of in-terventions, coordination of care, reinforcement of health teaching and promotion and evaluation of goals and interventions. 4. Maintains a safe, therapeutic, and healing environment through ensuring patient privacy and responsive, respectful, accepting, and professional behavior. 5. Utilizes and promotes the use of technology and equipment to enhance the safety of pa-tients and staff. 6. Communicates effectively to promote sharing of information throughout the continuum of care and as an effective member of the healthcare team. 7. Demonstrates the ability to function in a professional setting through active participation in a professional practice model. 8. Participates in shared governance, adherence to standards of care, participation in peer review and professional development promoting leadership and clinical excellence, ef-fective team skills, autonomous practice and acknowledgment of accountability for ac-tions and critical thinking. 9. Acts as a patient advocate throughout the continuum of care, including all patient set-tings and in the community. 10. Coordinates the effective and efficient delivery of patient care through the application of care management principles. 11. Ability to identify clinical priorities and is able to initiate actions, for example: emer-gent needs and/or care related to direct patient care. 12. Delegation to unlicensed assistive personal and other licensed practical nurses.
