Overview
JOB SUMMARY: The Ambulatory Care Pharmacist in the Pharmacy Care Clinic manages medication therapy for chronic disease states under collaborative practice agreements. These include anticoagulation, diabetes, hypertension, dyslipidemia, asthma, COPD, and tobacco cessation. The pharmacist in this position will perform point-of-care tests to monitor effectiveness of therapy (INR and HbA1c); educate patients and caregivers on medications and disease states; assess patient needs and provide tools to support the patient in their self-care. In addition, the pharmacist will precept learners (students and residents), participate in research projects; lead projects to improve quality of care; and participate on Medical Center and/or Department committees. To be successful in this role, the pharmacist must demonstrate a commitment to high quality patient care, ongoing learning, and skills development; must have excellent interpersonal communication, decision making, problem solving, supervisory, and customer services skills; must demonstrate a positive attitude, commitment to teamwork, attention to detail, and be flexible to provide services based on the daily needs of the team.
