Job Summary
Full Time; Monday-Friday 8:00am-5:00pm. Gaston County and Mecklenburg County. Location: Airport Campus, Charlotte, NC. occasional travel to clinics.
The Coordinator- Clinical Pharmacist provides patient-centered care to patients. Must provide pharmaceutical care to patients to achieve optimal therapeutic outcomes, promote cost-effective drug therapy, and evaluate clinical programs to ensure optimal pharmaceutical care for patients. Serve as a clinical preceptor for pharmacy students- providing support, education, and participation in independent or collaborative projects that improve patient care or resolve clinical problems. Participates in planning processes, implementation, as well as monitoring to support quality improvement efforts and contribute to the achievement of departmental objectives and population health initiatives
Essential Functions
Physical Requirements
Requires considerable walking, standing, pushing mobile cart of 50 pounds maximum and lifting 25 pounds of material. May be required to frequently stretch, bend, squat, or kneel to perform job. Utilizes appropriate personal protective equipment (PPE) to minimize exposure to hazardous agents. Takes necessary precautions to avoid inadvertent needle sticks, minor cuts or bruises. Exposed to various conditions throughout the workday such as walking on hard surfaces, climbing stairs and changes in temperature and humidity. Must be able to perform manipulative skills such as writing, typing, and data entry into the computer and automated dispensing machines. Must have use of both hands, be able to see clearly and read small type. Must be able to speak and be understood by others to communicate effectively. Must be able to hear normal sounds with moderate background noises. Involves rotating shifts.
Education, Experience and Certifications
BS in Pharmacy or Pharm D. from an accredited school of pharmacy required. Licensed in the state of North Carolina or eligible for reciprocity. Pharmacy residency training in the clinical area needed for the position or 2 years of experience as a clinical pharmacist required. Pharmacy Residency Program completion (PGY1) Board Certified Pharmacist (Accredited Board Certification).
