Overview
JOB SUMMARY:
Provides social work services, clinical intervention, complex treatment planning and care coordination to patients and families, when relevant. Additionally, offers counseling to patients with a variety of behavioral health needs and provides treatment recommendations to interdisciplinary team regarding behavioral health care. In conjunction with the multi-disciplinary team, assists with the development of a comprehensive treatment plan to meet short- and longer-term health and mental health needs.
EDUCATION/EXPERIENCE:
Master of Social Work from an institution accredited by the Council on Social Work Education.
Clinical social work experience with three years' experience working in a mental health setting with clinical diagnosis and treatment responsibilities required.
LICENSURE, CERTIFICATION, and/or REGISTRATION:
Clinical licensure in the state of North Carolina through the Social Work Certification and Licensure Board required.
ESSENTIAL FUNCTIONS:
1. Completes psychosocial assessment in accordance with departmental and professional standards to identify emotional, social and environmental needs related to diagnosis, illness, treatment and life situation.
2. Works with patients and families to provide clinical assessment and mental health intervention in conjunction with the health care team.
3. Formulates, develops and implements a psychosocial treatment plan utilizing appropriate social work modalities and interventions, which may include crisis intervention, individual and family therapies, and grief and bereavement counseling.
4. Conducts team meetings, in conjunction with physician and other team members, to review the treatment plan and to establish plans for treatment care.
5. Works in conjunction with the Psychiatric team to offer resource information and referrals for behavioral health care services beyond the scope of the provider's practice, such as for psychiatric medication evaluations.
6. Maintains knowledge of federal, state and local mental health system in order to facilitate ethical treatment planning and referrals. Upon request, consults with other mental health professionals and the health care team re: referral processes for community based mental health services.
7. Advocates for additional state and federal resources in the continuum of mental health services.
8. Documents according to Departmental standards.

SKILLS/QUALIFICATIONS:
Excellent verbal and written communication skills.
Demonstrates proficiency in the use of DSM-V criteria.
Proficiency using a personal computer.
Ability to work independently and collaboratively.

Embraces change in a rapidly changing health care environment.
Demonstrates knowledge of mental health system and reforms.
Social work experience in a healthcare setting desirable
