Job Summary
Under the direction of the NHA, assists all patients in dealing with social and related problems and manages age-appropriate activities program for all patients during skilled nursing stay. Must demonstrate effective communication, interpersonal, teamwork, organization, and prioritization skills. Is responsible for designated equipment and supplies used in the care of assigned patients. Must be flexible in changing assignments, workload and learning new skills. Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age and type of the patients served on his/her assigned unit.
Essential Functions
Implements admission, transfer, and discharge procedures, according to unit standards: Assures that admission documents are executed properly and timely. Assists with timely admission, discharge or transfer including follow-up appointments and arranging for durable medical equipment prescribed for home use.
Manages patient activities program that meets the needs of all patients. Delivers suitable recreational and entertainment activities for patients according to their needs and interests.
Participates in patient care meetings. Assures timely and appropriate communication of patient/ family issues. Establishes therapeutic relationships with patients, families, and members of the health care team through effective use of verbal and non-verbal communication skills. Utilizes appropriate lines of authority.
Education, Training, and Experience
High School Diploma or GED
Other Job Requirements, Licenses, Certifications
No licenses required.
CPR required.
Annual training will be arranged to ensure meets guidelines for activities and social services function.
