Overview

Job Summary 

The Assistant Vice President, Integrated Care Management/Centralized Transfer Center is an executive with accountability for developing services across the Atrium Health Greater Middle Georgia Area.  The role provides strategic leadership in developing health principles. Works closely with executive leadership to facilitate care redesign that engages care technology, health outcomes, and positions Atrium Health Navicent as a leader in value-based care. Scope of leadership includes care management (utilization management, discharge planning), non-emergent transportation, clinical documentation improvement, centralized transfer center and bed placement, and capacity management. 

Essential Functions 

  • Serves as a leader providing strategic direction and operational oversight for care management and patient flow. 
  • Oversight of comprehensive clinical care management programs, products, and policies including inpatient and observation care.  
  • Incorporates care management and patient movement information, data, and tools into workflows. 
  • Monitors performance in identified Care Management and Transfer Center metrics and facility quality metrics.  
  • Oversight of capacity management and patient flow within and between Atrium Health Navicent facilities.
  • Oversees human resource management for the assigned areas creating a culture of high employee engagement.
  • Organizes departmental areas through appropriate structure and delegation of functions. This function also requires operational and strategic coordination with other areas across Atrium Health including, but not limited to Clinically Integrated Networks, Quality, Pharmacy, Medical Group, and hospital operations.  
  • Participates in Atrium Health’s clinical integration efforts with existing and future partners. Identifies opportunities and synergies for enterprise alignment and integration and executes on those operations. 
  • Provides services and collaboration as necessary with regional partners.  
  • Assist in developing and meeting annual budgetary and organizational goals. Staff and operating budget management.  
  • Serves on or leads various committees to assist in achievement of strategic and operational goals.  
  • Assists the organization with successfully participating in the value-based transformation. 
  • Assures compliance with professional and regulatory practice standards.  
  • Analyze data to identify under/over utilization; improve resource consumption; promote health systems’ strategic goals and objectives.  

Education, Training, and Experience 

Master’s degree required.  Minimum of 10 years of progressive healthcare leadership career to include healthcare operational experience. Experience in logistics preferred, but not required.

Other Job Requirements, Licenses, Certifications 

Certification in Case/Care Management preferred, but not required. 

Physical Requirements 

Must be able to use visual acuity to see monitor screen, computer, and hard copy materials. Must be able to hear and verbally communicate in person and over the phone or radio. Must be able to sit for prolonged periods of time (up to 2 hours). Must be able to comprehend and learn operation of various office equipment. Must have functional range of motion of the cervical, thoracic, and lumbar spines, upper and lower extremities with a grip strength of 50-60# specific to job evaluation. Must be able to forward reach, overhead reach, bend, squat, kneel and apply proper body mechanics during the transfers and transport supplies and/or equipment using proper body mechanics. Must be able to lift to 10# specific to job evaluation.
