The APP will provide patient care and support medical education at Atrium Health’s Sardis Oaks, CMC Elizabeth Family Medicine, and a local continuing care retirement community.

Atrium Health Sardis Oaks Nursing Home provides skilled nursing and rehabilitative services for short-term, long-term and end-of-life care. CMC Elizabeth Family Medicine provides advanced family medicine care from birth throughout all stages of life and serves as a primary teaching site for future physicians. 
 

Highlights about the position include:

  • Full-Time (1.0 FTE) Opportunity - .6 FTE dedicated to providing patient care at Atrium Health’s Sardis Oaks; .4 FTE dedicated to providing primary care to patients of Elizabeth Family Medicine and a local continuing care retirement community
  • Schedule: Monday – Friday
  • No Call Responsibilities
  • Join a collaborating team of Family Medicine Physicians, Geriatricians, Advance Practice Providers, FM Residents and Geriatric Fellows, Medical Students, support staff, care management team, social work, behavioral health team, and pharmacy team.
  • Provide clinical care in a variety of settings including outpatient primary care and nursing home/assisted living facilities.
  • Teach geriatric medicine principles to Medical Students, Residents, and Geriatric Medicine fellows.

 Ideal candidate:

  • 3+ Years of experience in providing care in continuing care retirement communities, desired
  • 3+ Years of experience in ambulatory care settings and primary care geriatrics, desired
  • Interest in teaching and collaborating with resident physicians and fellows, medical students, and students in other health disciplines related to geriatrics and long-term care
  • Independent clinical assessment skills and decision making is required
  • Strong communication skills with patients, families, trainees, and interdisciplinary team members
