Overview

What you will do:

  • Coordinates and supports the providers and multidisciplinary team in facilitating patient care, providing ongoing care management services during an acute-care admission in which inpatient pediatrics was consulted including evaluating and triaging patients.
  • Provides discharge planning and post-acute care needs assessment for this patient population while serving as a liaison for discharge planning for patients’ pediatric needs.
  • Responsible for coordinating the activities of the pediatric hospitalist, physician assistant, and nurse practitioner including scheduling patients for procedures and coordinating the rounding activities for bedded inpatients.
  • Provides professional nursing care for assigned patient(s) in one or more patient care departments according to State Board of Nursing Practice Act, American Nursing Association Code of Ethics and standards of practice and professional performance.

What you will need:

  • Education: Completion of an accredited nursing educational program with a current registered nursing Georgia license, BSN required.
  • Experience: Minimum of three years of recent experience in pediatric care. Experience with IT solutions such as electronic health record, learning management or disease/care management systems a plus.
  • Certification/License/Registration: Georgia Registered Nurses License
