Post Acute Liaison | Rehab Hospital | PRN

Overview

Job Summary: Licensed clinicians who are responsible to help patients and their family with the transition to the next level of care (Inpatient acute rehab, outpatient rehab, swing bed, nursing home, home health, home hospice, inpatient hospice, etc.) Serves as an active advocate for the care transition needs of patient. Serves in organizational roles that help facilitate care transition needs and improve patient care outcomes. Work cooperatively coordinating post-acute care transition services and resources with the multidisciplinary team and Care Transitions Coordinators, while providing guidance to the Care Transitions team in an effort to appropriately transition patients in a timely manner.

Specifications: 1. Education: Must be a High School or GED Diploma Graduate. Bachelor’s degree preferred in health related field . 2. Experience: If a registered nurse 2-4 years of related work experience in a similar size, similar scope hospital preferred. If licensed and registered in another profession (i.e. therapist), must have 5 years experience in an inpatient setting. Previous experience assisting patients through the continuum of care and the transition from hospital to post acute care preferred. 3. Certification/License/Registration: Licensed in the state of Georgia
