Carolinas Palliative Care & Hospice Group (CPCHG), is currently seeking a Hospice Physician to join their growing hospice team in Cabarrus County, NC. 

Position Highlights:

• Work/life balance is prioritized and hours are Monday-Friday 8:30am-5:00pm in an outpatient setting.
• Rotation for night and weekends are every 7 to 8 weeks.
• Join interdisciplinary team (3 Physicians & 7 APP/s) with collegial relationships that foster team-oriented care.
• Serve as attending provider for the home care teams, hospice regulatory requirements including face to face visits in the home, symptom and initiation visits and symptom calls during working hours. 
• Maintain quality of life for the hospice patients and supporting their families and have a clear understating of the hospice philosophy.  

Ideal Candidate:

• Must be Board Certified in Internal Medicine or Family Medicine with significant experience in care of adults with acute and chronic illness.
• Board Certified/Board Eligible in Hospice & Palliative Medicine preferred but not required.
• Clean background and NC licensure required.
• Desired candidates have a strong passion for end-of-life care, can work well in busy environment, successfully able to manage timely documentation requirements and are very comfortable in working well as in integrated member of an interdisciplinary team with many other disciplines to achieve goals of care.
