Overview

Responsible for provision of care, health education and promotion and disease prevention for patients.  Care is delivered based upon established protocols. Duties will involve initial evaluation with a focus on medical history, physical assessment, education, disease management, ongoing monitoring and early interventions.  Provides primary care and patient education and prescribes patient care intervention to adults and families.  Additional duties include, but are not limited to, telephone consultations with Primary Care Physicians, routine assessment of patient health status, medication adjustments. Facilitates continuity of care delivery by serving as a practitioner, manager, consultant/leader, and change agent. Requires independent action, creativity, initiative, critical thinking, collaboration, interpersonal communication skills, and public relation skills. Position also requires a high degree of visibility, confidentiality, and flexibility. Supervisory/management ability necessary to provide organization and direction to patient care delivery. Works as liaison between patients, families, nurses, physicians, administration and other health care services and community service providers .

MSN required.  Five years nursing experience required.  One year as a nurse practitioner preferred.  Current APRN licensure with the Georgia State Board of Nursing
