Overview

Job Summary

Assists Continuing Care Services division as well as supports system initiatives in managing episodes of care in the post-acute setting. Transition Coordinator will assist in identifying, triaging, placing and tracking designated patients to get the most patients to the right places at the right time. Acts as a resource to post-acute care providers, patients, families and others while navigating the post-hospital stay transition. Transition Coordinator will also assist in promoting post-acute care service lines and ensure seamless patient care transitions.



Essential Functions

  • Works with Continuing Care Transitions team to help decrease ACO Spend
  • Aides in executing the “high value” post-acute network strategy
  • Assists case management in identifying specific patient populations and subsequently triaging those who have post-acute care needs
  • Supports transition to lower level of care and participates in discharge planning
  • Supports efforts to meet quality metrics by service line
  • Helps in tracking patients by providing early notification regarding risk for poor outcomes
  • Coordinates handoff to next level of care provider
  • Notifies primary care provider of patient’s involvement in shared savings program
  • Collaborates with the operations team to meet strategic goals that include growth, quality and service excellence
  • Participates in marketing events such as seminars, trade shows, health fairs and special events as appropriate

     

Physical Requirements
Work requires significant local travel and may require occasional overnight travel and weekend and/or evening work. Work involves liftin, moving and transporting large materials; walking, sitting and driving automobile for long periods of time. Responsible for the operation of equipment including computers, printers and safety equipment.



Education, Experience and Certifications

Bachelor's Degree in Business, marketing or healthcare related field required. 2 years’ experience in healthcare setting and/or healthcare sales with proven track record of success preferred.
