Registered Nurse | Hospice Case Manager | Days

Overview

The Hospice Case Manager provides direct nursing care to hospice patients, including needs assessment and developing, implementing, and evaluating a plan of nursing care for inpatient and home care patients.  Provides direct patient care on an ongoing basis in accordance with the Nurse Practice Act and Hospice standards of care. 

Position details: Weekday business hours with intermittent call shifts 

What you will need: 

• Associates or bachelor's degree in nursing 

• Current valid Georgia nursing license or eNLC nursing license 

• Two (2) years of nursing experience.  Hospice or Home Health experience and CHPN certification preferred. 

• Ability to work well within an interdisciplinary team setting as well as independently.
