Job Summary

The Atrium Health Human Trafficking Team seeks a dynamic, motivated leader to drive the anti-trafficking campaign across the system.  The Program Coordinator will provide complex case manager to survivors identified at CMC Main and LCH Hospitals (estimated 1-4 inpatients per week and up to 10 consultations for technical assistance per week).  The Program Coordinator will oversee the quality of responses and provide follow up to survivors served in consultation with the Abuse and Exploitation hotline or by the weekend CODE HOPE advocates.  Experience with victim advocacy, complex case management, and supervisory experience required.  The ideal candidate will enjoy caring for highly vulnerable persons, enjoy teaching others to build capacity, and see the value of data collection to drive programmatic improvement.  Salary commensurate with experience.

Essential Functions

• Provides and evaluates response to counseling/support/education services to patients/families receiving care by the Atrium Health Human Trafficking Team.
• Provides formal/informal education to staff and physicians/providers to help them better understand the emotional, social, and psychological impact of complex trauma and human trafficking on survivors and the effect on the family unit.
• Assists patients/families in arranging comprehensive services for mental health treatment, housing and emergency shelter, safety planning, legal services, and other multi-tiered aspects of a recovery plan.
• Reports suspected cases of child and/or adult abuse/neglect/exploitation. Serves as liaison between hospital, patient/families and Department of Social services during evaluation and investigation.
• Develops and maintains accurate case records of each referred patient.
• Leads multidisciplinary teams in care planning for the survivor as needed.
• Ensures accurate and timely data collection to drive programmatic improvement.
• Recognizes and provides for cultural, language, and age-specific differences of the patient/family.

   

Physical Requirements
Work requires walking, standing, sitting for long periods of time, lifting, reaching, and bending. Must be able to lift 75 pounds in weight, handling office equipment and supplies. Must have intact sense of sight, hearing, smell, touch, and finger dexterity. May require travel to current and potential provider/facility offices. (Use of personal vehicle required). Occasional intermittent noise and exposure to conditions such as dust, fumes, and chemicals.

Education, Experience and Certifications

Master's degree in Social Work from a program accredited by the council in Social Work Education required. Must adhere to the National Association of Social Workers Code of Ethics.
