Job Summary
Accountable for achieving effective and measureable clinical, financial, and functional outcomes. Expert clinician who works in partnership with the multidisciplinary team to coordinate development, implementation and evaluation of a holistic plan of care for the patient and/or family across the continuum of care. Utilizing evidence based practice, The manager will focus on larger populations for the purpose of improving clinical performance, patient care outcomes and resource utilization. Functions as a leader and/or supervisor in his/her specific assigned areas of responsibility.

Essential Functions



  • Utilizing evidence based practice, managers focus on larger population for the purpose of improving clinical performance, patient care outcomes, and resource utilization.
  • Analyzes and evaluates clinical data as it relates to clinical outcomes and resource utilization.
  • Develops process improvement goals and strategies.
  • Organizes and leads process improvement teams.
  • Develops standards of care/clinical guidelines/protocols in collaboration with multidisciplinary groups.
  • Serves as a clinical and systems expert resource.
  • Communicates information/outcomes regarding system issues and/or patient populations to appropriate individual or groups.
  • Provides direction to facilitate/coordinate the progress of patient groups along the continuum of care to achieve expected outcomes based on established guidelines or protocols.
  • Focuses the activities of the healthcare team on the achievement of optimal outcomes, resource utilization, clinical expertise and improvement strategies.
  • Incorporates population specific knowledge into practice area.
Physical Requirements
Works in an office type setting, extensive walking throughout the facility. Prolonged periods of sitting, reviewing medical records, and documentation. Repetitive wrist motion and occasional lifting of up to 35 pounds. Requires frequent verbal and written communication in English. Intact sight and hearing with or without assistive devices is required. Additional department specific physical requirements may be identified for unique responsibilities within the department by Leadership

Education, Experience and Certifications
BSN required, Masters preferred. Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency, meet the licensure requirements in your home state; or for Non-National License Compact states, current RN license or temporary license as a Registered Nurse Petitioner required in the state where the RN works. Two years experience required in Care Management at Atrium Health. Obtain and maintain professional certification in a relevant clinical specialty, per departmental policy. Obtain and maintain BLS certification.
